Follow the steps below to get the forms and documents you need to complete your registration and enrollment.
Download the forms and materials you need.
Download these forms onto your computer. (Click on the form and when it opens, save the form/document to your computer. When you fill it out, save it again.)
Some forms are fillable online. If a form is fillable, complete it and print it with all other documents.
If you need help downloading the forms or filling them out, please call your local program. The contact and number is listed in your email.
You may also choose to complete your forms at the program office, your CYP Professionals will be happy to help you.
If your confirmation email mentions a deposit, please make sure you pay within 48 hours. Your child's space is not guaranteed until you have paid your deposit. Refer to your email for details.
